Uploading Evidence
How to attach photos, documents, and notes as evidence when completing compliance tasks.
Overview
When completing a compliance task, you can attach evidence to support the completion record. Evidence creates an audit trail that demonstrates compliance during regulatory inspections. Depending on the task's configuration, some evidence types may be required.
Evidence Types
There are three types of evidence you can attach to a task completion:
Photo Evidence
A photo captured from your device's camera or uploaded from your files. Accepted formats are JPEG, PNG, and GIF. Photos are useful for:
- Documenting the condition of equipment
- Capturing test result displays
- Recording physical inspection findings
- Photographing expiration labels or tags
In the task completion modal, click the Add Photo button to capture or upload a photo. Once attached, the file name is displayed with an option to remove it.
Document Evidence
One or more documents uploaded as supporting evidence. You can attach multiple documents to a single task completion. Documents are useful for:
- Lab reports and test results
- Certificates and inspection reports
- Manufacturer compliance letters
- Maintenance service records
The document upload section appears below the photo button. Click to add documents, and each uploaded file is listed with its type.
Notes
Free-text notes entered in the Notes text area. Notes are useful for:
- Recording observations during an inspection
- Describing remediation steps taken
- Adding context about unusual circumstances
- Documenting follow-up actions planned
Evidence Requirements Configuration
Each compliance task can be configured with specific evidence requirements. These requirements are set per-task based on the compliance regulation and are stored in the task's evidence configuration.
The three configurable requirements are:

When a requirement is active, the corresponding field label shows an asterisk (*). The task completion modal will not allow submission until all required evidence is provided. If you attempt to submit with missing required evidence, an error toast appears listing the missing items (e.g., "Required evidence missing: photo, document").
Evidence requirements are configured per-task during compliance plan generation. You cannot change them from the completion modal. If you believe a requirement is incorrect, contact your practice administrator.
Evidence by Primitive Type
Different primitive types interact with evidence in different ways:
Heartbeat
- Notes and document upload are shown
- No photo upload option is available
- Evidence requirements can still apply (notes, documents)
Inspector
- All three evidence types are available (notes, photo, documents)
- The pass/fail result, lot number, and remediation note are recorded as structured evidence automatically
- Photo evidence is common for documenting test strip results
Meter
- All three evidence types are available
- The numeric reading is recorded as structured evidence automatically
- Photo evidence is common for documenting meter displays
Clock
- All three evidence types are available
- The expiration date or quick confirm status is recorded as structured evidence automatically
- Photo evidence is common for documenting expiration labels
Certificate
- All three evidence types are available
- The certificate upload is the primary evidence (handled by the primitive itself)
- Date of service, provider name, and next due date are recorded as structured evidence
- Additional notes and photos supplement the uploaded certificate
Auto
- No evidence collection -- tasks are completed automatically by connected equipment
- No user interaction is needed
Viewing Evidence After Completion
Evidence attached to completed tasks can be viewed in several places:
- Task Completion Modal -- Expand "Previous completions" to see evidence from past completion cycles
- Compliance History -- Click the eye icon on any completed task to view the full evidence detail dialog, including all attached notes, photos, and documents
- Inspector View -- Evidence counts are shown in the requirements table
Documents stored in evidence can be downloaded from the history detail view using the download button next to each evidence item.
